Community support/Case management
A Family Connections Community Health Worker (CHW) serves as a liaison between providers and other community supports that strengthen a client’s overall Behavioral Health Services.
A Community Health Worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, education, informal counseling, social support and advocacy.
Although responsibilities vary, here are examples of potential tasks completed by community health workers:
- • Assist in finding and establishing a relationship with medical and other behavioral health service providers.
- • Act as a liaison between various providers
- • Help navigate the health care system.
- • Find resources for clients (housing, food, supplies and more).
- • Provide support and education during pregnancy and patients with chronic disease.
- • Provides translation and transportation services
